Academic Forms

General Absence Form

All absences, for any reason, must be reported to the class's faculty member as soon as possible by the student, either in person, by phone or via email.  Students are always responsible for contacting their instructor(s) to get any material or assignments missed in class, even if there are important circumstances that prevent a student from attending.

STEM Course Approval Form

Approved STEM Electives are located on The Gatton Academy webpage.  Any course not already approved may be approved as a STEM Elective on a case-by-case basis providing content is sufficiently linked to a core math and/or science principle. 

High School Requirement Form (Non-WKU course)

Students may fulfill high school graduation requirements outside of Gatton/WKU with approval from the Coordinator for Student Success. You will need to receive approval prior to enrolling in the course.

Fifth Week Assessment Form

Conducted during the 5th week of each semester. Students are asked to take this form to all instructors for their 200-level and above courses. Dates of assessment will be provided via email from the Coordinator for Student Success.

College Forms

College Visit Absence Form

All college-related visits should be planned during weekends, summers, university holidays, and university breaks whenever possible.

Sometimes obligatory college tours, interviews, scholar days, or orientation days will inevitably fall on days when classes meet. If such visits will result in absence from any class, Gatton Academy students must have their absences approved in advance by the Coordinator for Student Success.

At least one week before missing class/es, please fill out the web form below. A letter will be generated in The Gatton Academy's office. You will be notified by e-mail when your letter is ready to pick up. You will then pick up and present your letter to all professors in advance of missing their class.

All students should work ahead to make-up any missed assignments, quizzes, exams, etc. before the absence from class occurs. Students with excessive absences may not be approved to miss additional classes. Academic standing will be considered before approval. Approved absences within this policy do not accumulate in The Gatton Academy Classroom Intervention Program.

Research Forms

Research Agreement

To be signed by Gatton Academy students and their research mentor at the onset of each semester of a student's research experience. Completed forms should be turned in to The Gatton Academy's Assistant Director, Academic Services.

Research Supplies Grant (RSG) Application

$250 grants given to the faculty mentor/department to be used for supplies a Gatton Academy student may need to conduct research. Applications are accepted throughout the year.  Research Supplies Grants funds are limited; application cycles start each July 1, and awards are made until funds are exhausted. Grants are given at a maximum rate of one per student, per academic year.
