Service Description: A physician-directed and nurse coordinated program of care that addresses the needs of all terminally ill patients and their families through patient and family support, training in caregiver techniques, aiding communication between patient and family, helping them to accept the reality of death, and Bereavement Program counseling for up to one year after the death of a patient
