In Network Insurance Companies
WKU Health Services will courtesy bill any insurance with the exception of Medicare or Medicaid. We will bill HMO plans, however, these plans normally require pre-authorization or a referral from your Primary Care Manager(PCM) which is the responsibility of the member/patient to receive. Payment and coverage amounts will differ according to the insurance company's plan/benefit design and out-of-network benefits, therefore payment from these plans is not guaranteed.
At the time of service please be prepared to pay your co-pay, provide a copy of your
insurance card and present a form of identification.

1 With Aetna HMO plans the insured needs to go to their employer and get an enrollment form and say they are an "out of area dependent."
2 Blue Cross/Blue Shield has a variety of plans. It is advised that you call your plan provider and determine if you need a new card.
3 CIGNA - If the insured has an out-of-state HMO plan they need to contact CIGNA to see if their plan offers Guest Privileges.
4 Ft. Knox (Ireland Army Hospital) and Ft. Campbell (Blanchfield Army Hospital) will no longer be issuing referrals for Tricare Prime patients to Primary Care facilities such as WKU Health Services. If your Primary Care Manager(PCM/family doctor) is located at either of these facilities, Tricare will ask you to pay a percentage of your visit charges instead of a copay. These facilities are requesting you be seen at their office or have your PCM changed to a local provider. For more information, please contact your PCM.
