Faculty/Staff please bring your insurance card and a photo ID along with you if you wish to take advantage of this opportunity. If you are covered by the WKU Health Plan this service is covered at 100% with no co-pay.
Students please bring your insurance card and a photo ID along with you if you wish to take advantage of this opportunity. Your insurance may not cover this service 100%, however we will courtesy bill them and you will be responsible for any remaining balance.
Community/Non-insured please bring a photo ID along with you. The cost is $15.00. We accept cash, check, Big Red Dollars, Visa, Master Card and Discover.
