If your card is lost or stolen, be sure to contact the ID Center to have your card deactivated ASAP! You can call (270) 745-2417, come by the ID Center in DUC 126, or use the ManageMyID.com site to deactivate the card. You may purchase a replacement card for $20 (this will automatically have all of your old card’s balances and priviledges encoded on it and will automatically deactivate your old card).
If your card has been damaged due to normal wear and tear, bring it in (all the pieces if it is broken), and you will receive a free replacement (only one free replacement per year allowed). However, if your card has been cut, hole-punched, or suffers from any visible misuse, replacements can be purchased for $20.
Cardholders are only allowed to keep one active card in their possession at a time.
