SNAP Benefits Form
Students and parents of dependent students who are selected for Verification Group 2 or Group 4 are required to complete a SNAP Benefits Form to indicate whether a member of the household received benefits from the Supplemental Nutrition Assistance Program or SNAP (formerly known as the Food Stamp Program) at any time during the previous two years.
Sign in to Zip Access at www.kheaa.com. Choose "KHEAA Verify." Click on the Dependent or Independent* SNAP Benefits Form that was listed in the MIL you received from WKU.
*Students who were required to include parent information on the FAFSA will need to complete the Dependent SNAP Benefits Form. Students who were not required to provide parent information on the FAFSA will need to complete the Independent SNAP Benefits Form.
