An academic plan must be submitted to Graduate Studies and Research by students who are appealing dismissal. Students on probation may also be asked to submit an academic plan by his or her department.
Authorization for Use is submitted to the Graduate School along with the thesis, specialist project or dissertation.
Certificate Program Forms outline a Program of Study for a graduate certificate and should be on file upon admission to the certificate program.
Any change in the approved Program of Study must be specified on a Course Change Form. (If the student is seeking teaching certification or a rank change, he/she must upload the form at the Teacher Certification website.)
In specific programs students may choose to complete the research tool requirement by taking a foreign language.
Program of Study (Form C-Master's and Specialist Degrees) or Program of Study (Form C-Doctoral Students)
All degree-seeking students must submit a planned Program of Study (Form C) to the Graduate School within the first semester (or up to 12 credit hours) of course work (inclusive of transfer credit). The degree program is to be developed in consultation with and approved by the advisor assigned at the time of admission. (If the student is seeking teaching certification or rank change, he/she must upload the form at the Teacher Certification website.)
Admission to candidacy is a traditional component of graduate study, the purpose of which is to provide a planned, formal review of the student’s progress toward the specified program of study. Admission to the Graduate School and admission to program candidacy are two separate procedures.
All dissertation students (and beginning in Fall 2014 all thesis students) should submit a Committee Membership (Form G) to the Graduate School prior to enrolling any 599, 699, or 799 course. EdD students should complete this step upon completion of 21 required core course credits.
This agreement outlines the details of the students assistantship and serves as a contract for the student and the department which has appointed the assistantship. The form is to be completed by the hiring department.
Funding from the Provost's Initiatives for Excellence and the Graduate School makes this conference and research funding possible. Travel funds are intended for student traveling to professional meetings and conferences and for students who need to travel to conduct research. A budget must also be submitted. Teachers serving as student teaching supervisors are eligible for six hours of tuition-free courses for each semester of supervision during an academic year. Intern resource teachers are eligible for six hours of tuition-free courses for each year they serve as a resource teacher. Submit this form to the College of Education and Behavioral Sciences Dean's Office.
