History Contest Entry Form

Please fill out the following information. If you have more than five students, be sure to list the additional students in the box at the bottom, PLUS the test that each one will be taking: Junior High American History (JRA), Senior High American History (SRA), Junior High World History (JRW), or Senior High World History (SRW). This form will be mailed automatically to the History Contest Coordinator, Dr. Tamara Van Dyken; please contact her via email at wkuhistorycontest@wku.edu if you have any questions.
