PROXIMITY CARD REQUEST FORM
ONLY AUTHORIZED INDIVIDUALS AS PREVIOUSLY PROVIDED TO THE ID CENTER MAY USE THIS FORM. ANY OTHER REQUESTS WILL BE DISREGARDED.
These cards are $5.00 each, and payment is due upon pickup. Any individuals receiving a proximity card must first turn in their current ID if they have previously been issued one.
You will be contacted when your order is ready.
*Requests made during semester start-up periods and/or ATP dates will not be processed until a later date.
