The Form D (Admission to Candidacy) follows completion of Form C. Completion of the Form D is essentially a two-step process.

1.  Completion of the form itself (which can be found at the following url):


This is a fairly straight-forward process. One must possess at least a 3.0 GPA in order to qualify for candidacy.   The form should be completed utilizing a 30-hour program and the number of hours completed to date along with the respective GPA in all graduate coursework.

The student portion should look similar to the example below:

Be sure to save and sign the form digitally before sending!!!!!


2.  Simultaneous completion of a course-change form (which is accessed at the following url):


The course change form provides up-to-date revisions from the original Form C filed.   For example, if a student listed COMM 565 as an elective but has opted to take a separate course in its place – the appropriate course listings would be included in the course change form. This must be done in order to get the Form D completed.

As a special note to all students who filed a Form C prior to June 1, 2012 – the program is now a 30-hour program, not 33 hours. As such, you may delete one elective course from your Form C; designation of that course would be completed with the course-change form.

This form does NOT need to be signed digitally prior to sending.
