WKU Health Services Provider Information

Dear Patients of WKU Health Services:

This letter is to inform you that WKU Health Services will cease operations effective close of business on July 31, 2014.  Our last day of seeing patients will be July 24th.

The University has chosen Graves Gilbert Clinic to operate the health services clinic on campus and they take over on August 1st. The physicians and nurse practitioner currently employed at WKU Health

Services will not be continuing to provide services through the private entity.

 

We are honored that you have chosen us for your health care.

After July 23rd if you wish to receive a copy of your records, please direct your request to:
