Online Forms:

Work Order and Key Request forms are now available online. Click the SimpleK® link below, and then pull down the "grayed" Request & Orders tab located at the top of your screen directly above the red WKU logo.  The Authorized Agent form will need to be on file with Access Control for you to use the online forms.

 

Not set up for SimpleK® online?

Download the Authorized Agent form located in the link at the botoom of this page and submit approved forms to Access Control Department. Completed forms can be dropped off in person at the WKU Police Access Control office,  PSI / Annex 2 next door to the  WKU Police Department.  Scanned copies may also be e-mailed to Accesscontrol@wku.edu
