Insurance Information: The flu vaccine is covered fully by the WKU Anthem BCBS plan and you will not be charged a co-pay. If you have insurance other than WKU Anthem BCBS, we can bill your insurance, but we cannot guarantee it will be covered. You may be subject to paying a co-insurance, deductible or co-pay amount depending on your individual plan. Currently we are unable to accept or bill Medicare or any state Medicaid insurance plan at this time. Please bring an I.D. and your insurance card if you want us to bill your insurance. Retirees: We do not participate with Medicare and are unable to bill them directly. If you have the Humana KTRS plan (Group # Q0725 or R0380), we are able to bill this plan directly. Patients on this plan have a $150 deductible, once they meet this, the flu shot is covered at 96%. A co-insurance of 4% will be billed to the patient after the claim has processed, unless the patient has already met their maximum out of pocket of $1200. You can pay cash if you choose to not bill insurance.
Payment Methods: If you are not filing insurance, you can pay with cash, big red dollars, check or with a credit card. Non-insured patients are eligible for a cash payment prompt pay discount.
