Nomination Form
To be eligible for nomination:
- Must be an Active, Full-time Facilities Management employee
- Monthly Awards: minimum ninety (90) days of service
- Annual Awards: minimum twelve (12) months of service
- No disciplinary actions on record within the past twelve (12) months
- No safety incidents within the past 12 months
- Monthly/Quarterly winners will also receive a nomination for the respective Annual
Award

CRITERIA's
Exceptional Job performance
✓Employee consistently receives praise for his/her exceptional service to the WKU
community
✓Employee acknowledges and takes pride in ownership of the day-to-day processes for
which he/she is responsible
Attendance
✓No missed/late punches
✓Zero unexcused absences for month
✓Volunteers for overtime/events
Attitude
✓Exhibits a Can-Do attitude, even under difficult/stressful circumstances
Innovative thinking
✓Finds creative ways to solve challenges, increase efficiency or lower costs
Teamwork
✓Goes out of his/her way to assist team members
✓Voluntarily pitches in to help team after their daily tasks are complete
Above and Beyond behaviors
✓Goes out of their way to solve customer requests and/or issues (within scope of service
level agreement)
✓Completes additional work/projects without being asked
