Registration Fee By submitting this form, you will be responsible for a $100 registration fee. This covers the processing and administrative work for your registration. If you decide to withdraw after submitting this form, you will still be responsible for paying the $100 registration fee. This fee will count towards your total tuition, so the total amount due for the semester will remain unchanged, as listed here: https://www.wku.edu/strings/tuition_and_policies.php
Tuition Payment Instructions All payments must be received by the WKU Cashier's Office by February 15, 2024 to avoid a late fee of $100. If paying by check, please factor in two extra weeks for processing. The following are instructions regarding ONLINE PAYMENT from the Cashier's Office: The Cashier’s Office has implemented a new process for credit card payments. This should be easier for anyone with an invoice to make payments. You will go to: https://app.wku.edu/pay-my-invoice/ to make a payment. Please have your Invoice Number, which begins with S0; Customer ID, which begins with 80; and the amount the invoice was originally written for, on hand.
Once the above information is input, you can “Lookup” your account and pay your outstanding invoice.

Review Your Registration Form Please review and check that all the information you submitted is correct, including name, email address, street address, lesson length (check with your teacher), etc. There will be a fee of $50 if any changes are needed after you submit the form.
Policy Information By registering my child for this program, I understand that photographs and videos may be taken to document activities. I give my permission for photographs and/or videos to be taken of my child during the semester to be used for educational and/or promotional materials for the Department of Music and its faculty.
