This form is for ON DEMAND COURSES ONLY. To withdraw from semester-based courses, please login to your TopNet account.

If you decide to withdraw from on demand courses, please fill out the request form below. (Items noted with an asterisk* are required.) Please allow 3 business days for our office to process your request.
A "W" will appear on the transcript if the student withdraws from the course within the first four months of enrollment. Please be advised that there may be a withdrawal fee assessed to the student if they withdraw after the first week. A student may not withdraw from their course after the first four months of enrollment or after having taken their final exam or completed all the course work required.
