APR Dean's Final Summary Report Template Template for deans to draft response to programs and prepare for the meeting with faculty; based on the self-study and other documentation and the conversation with the department, deans ultimately finalize the report with this template and submit it in Anthology Portfolio.

Creating a Review Committee & Selecting External Reviewers for Non-Accredited Programs Deans collaborate with department chairs to create review committees; Department chairs nominate discipline-specific reviewers who are external to WKU and check their availability and interest; deans select members internal to WKU but external to the discipline; deans ultimately select and charge review committees after consulting with department chair.
External Review Committee Membership Form This form should be completed and sent to rob.hale@wku.edu by November 22; receipt of the form will trigger the financial piece of the process for external reviewers.
Review Committee Initial Charge Letter from Dean Template After dean has consulted with department chair on committee membership, department chair has confirmed availability of external reviewers, and dean has confirmed availability of WKU members, dean sends this memo.

Dean’s Email Sharing Link to Self-Study with Review Committee Once the dean’s office has accepted the self-study and supporting documents as complete, by January 31, they should adapt this email to share the link to the self-study with all committee members (external to WKU and external to department). Please cc Rob Hale and the department chair/director with the email.
Sample Itinerary for Site Visit Department chairs are responsible for organizing the site visit, collaborating with Dean’s Office and Provost’s office on availability of personnel.
